Done completely in photoshop. I was really hungry when I did this, and surprisingly I did this one in about 30 minutes. Apparently really inspired. Hunger can do lots of crazy things to you. But anyways, I always envision intestines as a clumb of writhing, squirming, ravenous, worm/ snake like creatures that just get voracious when they're hungry and like start jumping and hissing at you, at times trying to tear themselves out so they can go hunt food down on their own, or maybe even squirm their way out your throat for the same purpose of hunting down a suitable meal for their unreasonable hunger. So here's the result of that... Hungry... and... omg... they're making they're way out...
